Job Summary
In-Home Dialysis Care Partner to support a female patient in her 60s during home hemodialysis. Provide one-on-one in-home assistance, including dialysis setup (Tableu or NxStage), keeping the environment clean and safe, offering companionship, helping with treatment scheduling, light ADL support, monitoring safety, reporting concerns to the coordinating nurse, and maintaining simple documentation. Training will be provided by the clinical team. Schedule is Mon, Tue, Thu, Fri from 10:00 AM to 1:00 PM at the patient’s home in St. Peters, MO. Ideal candidate has 1+ year dialysis experience and is comfortable working independently within a care network.
Required Qualifications
- 1+ year of dialysis experience (home or clinic)
- Familiarity with home dialysis systems preferred (Tableu or NxStage is a plus)
- Open to in-home dialysis support and willing to complete the provided training
- Strong communicator who works well with a clinical team
- Must pass a Herewith background check
