Job Summary
Care Partner needed to support a patient transitioning to home-based dialysis, providing one-on-one care, setup and monitoring of HomeChoice Claria APD or NxStage machines, maintaining a safe and organized treatment environment, offering companionship, helping with light ADL tasks, ensuring adherence to treatment times, reporting concerns to the coordinating nurse, and maintaining simple documentation and shift notes. Training and clinical team support are provided; the role emphasizes patient dignity, comfort, and continuity of care in the home setting.
Required Qualifications
- 1+ year dialysis experience (home or clinic) or Healthcare background (CNA, MA, or caregiver)
- Familiarity with home dialysis systems preferred (HomeChoice Claria APD or NxStage is a plus)
- Open to in-home dialysis support and willing to complete the provided training
- Must pass a Herewith background check
