Job Summary
In-Home Dialysis Care Partner to support a single client in Peoria, AZ, transitioning dialysis from clinic to home. Duties include setting up dialysis equipment (HomeChoice Claria APD or NxStage), maintaining a clean and safe treatment environment, offering companionship, keeping patient on treatment schedule, providing light ADL support, monitoring safety and reporting concerns to the coordinating nurse, maintaining simple documentation and shift notes, and collaborating with a clinical team. Requires 1+ year of dialysis experience (home or clinic) or healthcare background (CNA, MA, or caregiver); training will be provided.
Required Qualifications
- 1+ year of dialysis experience (home or clinic) or Healthcare background (CNA, MA, or caregiver)
- Familiarity with home dialysis systems preferred (HomeChoice Claria APD or NxStage is a plus)
- Open to in-home dialysis support and willing to complete the provided training
- Strong communicator who works well with a clinical team
- Must pass a Herewith background check
