Job Summary
Care Partner providing one-on-one support for a patient transitioning to home dialysis in Peoria, AZ. Assist with dialysis treatment setup (HomeChoice Claria APD or NxStage), keep the treatment environment clean and safe, offer companionship, help maintain treatment schedules, provide light ADL support, monitor safety, report concerns to the coordinating nurse, and maintain simple documentation and shift notes. Training will be provided by the clinical team to ensure confidence in home-based treatments and routines.
Required Qualifications
- 1+ year of dialysis experience (home or clinic) or Healthcare background (CNA, MA, or caregiver)
- Familiarity with home dialysis systems preferred (HomeChoice Claria APD or NxStage is a plus)
- Open to in-home dialysis support and willing to complete the provided training
- Strong communicator who works well with a clinical team
- Must pass a Herewith background check
