Job Summary
Provide one-on-one in-home dialysis support for a single client, including setting up dialysis treatment (HomeChoice Claria APD or NxStage), keeping the treatment area clean and safe, offering companionship, ensuring treatment scheduling adherence, providing light ADL support, monitoring safety, reporting concerns to the coordinating nurse, and maintaining simple shift notes and documentation. Training is provided and ongoing clinical team support is available to help ensure comfort and confidence during the home-based dialysis transition.
Required Qualifications
- 1+ year of dialysis experience (home or clinic) or Healthcare background (CNA, MA, or caregiver)
- Familiarity with home dialysis systems preferred (HomeChoice Claria APD or NxStage is a plus)
- Open to in-home dialysis support and willing to complete the provided training
- Strong communicator who works well with a clinical team
- Must pass a Herewith background check
