In-Home Dialysis Care Partner (1:1 Client) – Fluent Spanish Required

In-Home Dialysis Care Partner provides safe, consistent dialysis support for a single client in the patient's home. Responsibilities include assisting with in-home dialysis treatment setup (NxStage or similar systems), maintaining a clean and safe treatment environment, helping the patient stay on schedule with treatments, providing companionship and emotional support, assisting with light ADLs as needed, monitoring safety and reporting concerns to the coordinating nurse, maintaining simple documentation and shift notes, and collaborating with the clinical care team.
