Job Summary
Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, performing medication reviews, physical assessments, and diagnostic tests like diabetic eye exams or spirometry. Maintain at least 12 hours of weekly availability with a target of 50+ capacity units and 35+ completed visits monthly. Travel locally within designated metro areas, including Springfield MA, and accommodate up to 25% travel to licensed states. Perform video visits as needed while maintaining compliance with all training and diagnostic equipment requirements. Work Monday through Sunday with most visits scheduled between 8 a.m. and 7 p.m.
Required Qualifications
- Board-certified nurse practitioner
- Active state licenses in coverage areas (FNP, AGNP certification preferred for NPs)
- Maintain board certification
- Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
- Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies
- Be flexible to travel locally and within licensed states, as needed (up to 25%)
Desired Qualifications
- Maintain multi-state licenses (preferred)
- Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
- AGNP certification preferred for NPs
