Job Summary
Lead and oversee the CASL Community Care Program (CCP) In-Home Services, ensuring timely, high-quality service delivery for older adults and coordinated care with families, caregivers, and partners. Drive program performance through staff supervision, scheduling, intake and care plan implementation, workforce development, compliance with CCP requirements, and data-driven decision making. Build and maintain partnerships with care coordinators, referral partners, and external agencies; monitor quality assurance and service outcomes; ensure effective communication with clients and families; manage operational efficiency and workforce stability; and stay informed on regulatory changes affecting CCP services.
Required Qualifications
- Bachelor's degree in Social Work, Healthcare Administration, Public Administration, or a related field (preferred)
- 5–7 years of experience in home and community-based services, aging services, healthcare operations, or related field (preferred)
- Experience in operations, workforce scheduling, and service delivery in a high-volume environment (preferred)
- Strong knowledge of Medicaid-funded programs, particularly Illinois CCP (preferred)
- Experience with data management, reporting, and analytics (preferred)
- Experience building relationships with clients, families, referral partners, and stakeholders (preferred)
- Proficiency with Microsoft 365 applications (preferred)
- Experience using EVV systems or similar service-delivery platforms (preferred)
- Bilingual or multilingual skills, particularly Chinese or other Asian languages (strongly preferred)
