Job Summary

Identify and engage eligible patients for Advanced Primary Care Management enrollment during in-office visits, explaining care management benefits and conducting social needs screenings for barriers like transportation or housing instability. Serve as the in-office liaison between Phamily's Care Management team and clinical staff, providing real-time updates on patient enrollments and SDoH interventions while offering resource referrals for complex cases. Track key metrics including interactions, completions, and provider engagement to report progress. This role combines clinical empathy with operational execution to scale partnerships and embed care management as a core component of every patient's healthcare journey within Medical Group practices in Albuquerque, NM.

Required Qualifications

  • Master's Degree in Social Work (MSW)
  • Active Licensed Clinical Social Worker (LCSW) or equivalent state licensure
  • 3+ years in a healthcare, care management, or clinical social work setting
  • Proven ability to communicate effectively with physicians, practice staff, and patients
  • Demonstrated success in patient engagement, motivational interviewing, and addressing SDoH needs
  • Strong interpersonal and communication skills, with ability to build trust quickly
  • Comfort working in fast-paced, dynamic environments and adapting to evolving workflows
  • Proficiency with EHR systems (Epic, Athena, etc.) and care management platforms
  • Excellent organization and documentation habits
  • Empathetic, professional demeanor aligned with Phamily's mission of patient-centered care
  • Proven success in a high-performing environment: high velocity, strong ownership, comfort with ambiguity, resilience, and a true growth mindset
  • You are both a playbook builder and executor, able to design scalable approaches for today while anticipating what the business will need tomorrow, and then follow through to deliver results

Desired Qualifications

  • Active Licensed Clinical Social Worker (LCSW) or equivalent state licensure strongly preferred in Illinois
  • Experience working in primary care or value-based care environments preferred
