Job Summary
Registered Nurse Case Manager in Central Indiana responsible for coordinating care for a diverse patient caseload. Works with patients, families, PCPs, therapy providers, and internal teams to develop and implement individualized care plans, provide education, coach Home Health Aides to maintain high-quality patient care, and ensure timely clinical documentation. Roles include on-call rotation (approximately one weekend per month), potential holiday rotation, and phone triage night coverage. Requires Indiana RN licensure, 2 years of clinical RN experience with 1 year home health preferred, and familiarity with OASIS preferred. Full-time position with competitive compensation and benefits in Indiana-based home care coordination.
Required Qualifications
- Associate's Degree or Baccalaureate School of Nursing Degree
- Current and valid RN license in Indiana
- 2 years RN clinical experience; 1 year home health preferred
- Case management/supervisor experience and pediatric/adult care experience preferred
- Knowledge of Medicaid paid family caregiving program and HHA scope of practice preferred
- Reliable transportation, valid driver's license, current auto insurance
- Ability to work flexible hours, travel locally, provide on-call service
- Knowledge of OASIS preferred (training provided)
