Job Summary
Coordinate outreach to homeless populations and provide hands-on care-management support in community settings. Responsibilities include conducting intake assessments, developing and monitoring service plans, coordinating transportation, facilitating group sessions, advocating for clients, and maintaining client/staff communication with strict confidentiality. Work alongside law enforcement partners to connect individuals with services, promote self-determination, and ensure services align with funding and policy requirements. Requires a high school diploma or equivalent and at least two years of relevant experience; valid driver’s license; ability to pass a criminal background check; bilingual ability is a plus.
Required Qualifications
- High school diploma or equivalent
- Minimum of two years work experience in a relevant field
- Direct experience in working with homeless population and individuals who suffer from substance abuse and mental health issues
- Effective written and verbal communication skills
- Ability to pass a criminal background check
- Valid driver’s license and ability to meet agency driving standards
- Bilingual is a plus
