Job Summary
The IMPACT Specialist will coordinate care management for vulnerable populations by assessing emergency needs, providing direct services, and working alongside law enforcement. Responsibilities include facilitating street outreach, disseminating information about agency services, coordinating transportation, conducting intake assessments, and developing personalized service plans. The role requires effective communication with clients and community agencies, completing reports, facilitating group sessions, and providing necessary assistance to clients entering the facility. Essential skills include the ability to support clients in their daily living needs and maintain confidentiality.
Required Qualifications
- High school diploma or equivalent
- Minimum of two years work experience in a relevant field
- Direct experience in working with homeless population
- Experience with individuals suffering from substance abuse and mental health issues
- Effective written and verbal communication skills
- Ability to interact appropriately with clients
- Pass a criminal background check
- Valid driver’s license and ability to meet agency driving standards
Desired Qualifications
- Bilingual is a plus
