Job Summary
Evaluate patients' mobility, strength, range of motion, and pain levels to develop individualized treatment plans. Educate clients and caregivers on the proper use of assistive devices and exercises while delivering compassionate, client-centered care. Monitor and report patient progress or changes in condition to the Director of Nursing and physician, maintaining accurate documentation including progress notes and discharge planning. This per diem role offers flexible scheduling, the ability to choose caseload, and one-on-one in-home patient care for BrightStar Care clients.
Required Qualifications
- Degree in Physical Therapy from an accredited program
- Current state Physical Therapist license and/or successful completion of NPTE (or state exam)
- Minimum of 1 year of Physical Therapy experience
- Strong understanding of HIPAA compliance and commitment to client confidentiality
