Job Summary
Care Coordinator responsibilities include coordinating care for all patients seeking treatment, performing intake screenings via face-to-face or telehealth to determine level of care, administering biopsychosocial assessments in SAMMS with clinical justification, completing intake paperwork and admission notes within one business day, documenting with SOAP notes, coordinating admission with the Retention Specialist to ensure all steps (toxicology, medical team/nursing) are completed, and communicating with staff/referral sources. Schedule is Monday–Friday (7:00a–3:30p) with one weekend day weekly at the Manteno, IL location. Requires HS diploma/GED and IAODAPCA CADC certification (or ability to obtain within allotted time) and DCFS clearance; comprehensive benefits for full-time employment.
Required Qualifications
- High School Diploma or GED
- IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) required, or able to obtain within allotted time of hire date
- DCFS clearance required
