Job Summary
Care Navigator for Net Health/Limber Health delivering remote monitoring and guidance for patients in a home-exercise program. Provide remote instruction in therapeutic exercise, neuromuscular re-education, and functional training through our platform; give ongoing feedback to the evaluating clinician; maintain records; organize and manage a caseload; assist in improving platform processes. Must be a PTA with an accredited program graduation and hold an Illinois PTA license and an active compact-state PT license. Strong communication, tech proficiency across multiple platforms, and a patient-focused approach are essential.2+ years in musculoskeletal health is a plus, with a proactive, independent, and collaborative work style in a fast-paced healthcare SaaS environment.
Required Qualifications
- Graduated from an accredited physical therapy program
- Active PTA license in Illinois
- Active PT license in a compact state
