Conduct remote and on-site rounds for named care homes, managing primary care for elderly residents. Complete weekly phone visits to facilities, consult with caregivers on acute cases, and process administrative paperwork. Perform scheduled in-person visits to facilities every two months. Work primarily remotely with occasional on-site rotations. This role is part of Terveystalon public services, providing cost-effective patient-centered care across Pohjois-Karjala. The position utilizes the Mediatri patient information system.
