IIWCC/SWAN Registered Practical Nurse

Job Summary
Provide wound care and continence care to home-care clients; utilize the nursing process to deliver high-quality wound care in the community; collaborate with interdisciplinary health team to implement cost-effective, evidence-based wound care plans; develop and modify plans of care in consultation with physicians, CCAC Case Manager, ICCP coordinators, and Primary Nurse; promote client and caregiver independence; educate clients and families using adult learning principles; perform pre-visit assessments, review medical information and orders, document charting and visit verifications, and participate in bi-weekly nursing case rounds; contribute to policy and practice reviews and mentor other team members on wound care protocol and product usage; maintain ongoing communication with supervisors and participate in discharge planning, teaching, and care coordination; role requires BSN or PN Diploma and professional certification in wound care, with multiple credentials preferred (IIWCC, NSWOC, SWAN) and experience in home/community care.
Required Qualifications
- Bachelor’s of Science in Nursing or Diploma in Practical Nursing
- Registered with the College of Nurses of Ontario (CNO)
- Current BCLS/HCP certification
- Certificate in Wound Care (required)
- Experience: 1–3 years of clinical experience (required)
- Experience in complex wound care (required)
- Experience with IV medication administration (required)
