Job Summary
Meet with, set up, and provide ongoing training and support to individuals accessing the Individualized Home Supports with Training program. Travel to clients' homes in surrounding Metro counties to deliver these services. Maintain a valid driver's license, clean driving record, and reliable transportation for independent travel across Accra's geographic service areas. Complete a background study with the Minnesota Department of Human Services. This role supports Accra's mission to improve lives by providing individualized homecare services and support to people living at home.
Required Qualifications
- Bachelor degree in a field related to Human Services, Education, or Health
- one year of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older
- Associate degree in a field related to Human Services, Education, or Health
- two years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older
- Diploma in a field related to Human Services, Education, or Health from an accredited postsecondary institution
- three years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older
- Minimum of 50 hours of education and training related to Human Services and disabilities
- four years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older
- 1 year experience in a Human Service field
- Demonstrated creativity, critical thinking, and problem-solving skills
- Excellent organizational, problem solving, communication, and interpersonal skills
- Must pass a background study with the Minnesota Department of Human Services
- Have a valid driver's license
- clean driving record
- reliable transportation to use for work driving purposes
- Ability to travel independently across the Accra geographic service areas
