Job Summary
IHS Case Manager position with Lifeworks in Norwood, MA. Responsibilities include providing care and assistance to individuals living independently in the community, assisting with banking, budgeting, scheduling appointments, coordinating healthcare, facilitating one-on-one and group activities, promoting health, ensuring safe residential environments, delivering education and counseling to help achieve ISP goals, developing and presenting annual ISP and semi-annual assessments, coordinating transportation, and maintaining confidential files. Requires experience with ID/DD, a bachelor's degree, strong communication skills, and the ability to pass background checks and drive. Equal Opportunity Employer.
Required Qualifications
- Experience working with individuals with ID/DD
- Bachelor's degree from four-year college or university
- Valid Driver's License
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
