Job Summary
Provide individual, group, and family mental health services, case management, transportation, and other professional services to youth and families in the community; travel daily to community-based settings; meet or exceed productivity thresholds; participate in on-call crisis support and crisis intervention for IHBT families; coordinate and monitor services across mental health, education, medical, vocational, and social services; review and modify youth-based plans to ensure progress; establish and maintain strong relationships with foster parents and families; assist in family reunification and stabilization efforts; provide safety assessments, crisis response, and behavioral stabilization as required; participate in Continuous Quality Improvement activities and weekly supervision; maintain thorough documentation.
Required Qualifications
- Master’s Degree in Social Work or comparable Human Services field from an accredited institution
- 2 years of work experience working in direct service with youth and families strongly preferred
- A valid advanced professional license as required and acceptable by state or contract regulations
- Must have a valid driver’s license, reliable transportation, automotive insurance, and a good driving record
- A willingness to work flexible and non-traditional hours in the service of foster caregivers, families of origin, and persons served
