Job Summary
Lead admissions and census management by conducting pre-admission screenings, meet-and-greet visits, and developing initial Person-Centered Plans in collaboration with the interdisciplinary team. Coordinate service delivery through regular site visits to homes and day programs, monitoring PCP implementation, safety, and community integration while reviewing quality assurance data. Serve as the primary coordinator of the IDT by facilitating interdisciplinary meetings, interviewing individuals to define goals, and advocating for their rights and preferences. Maintain full bed occupancy by actively managing referrals and follow-up efforts, ensuring timely documentation and regulatory compliance across Medicaid and ICF requirements.
Required Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Special Education, or a related field
- At least 2 years of experience (in North Carolina)
- 1 year of experience (in Tennessee) working with individuals with intellectual and developmental disabilities
- 1 year required if Master's degree is held
- Demonstrated experience in developing, writing, and implementing Person-Centered Plans
- Working knowledge of ICF regulations, person-centered planning principles, and applicable Medicaid/HCBS rules
- Excellent communication, organizational, and documentation skills
- Valid driver's license, auto insurance, and reliable transportation
- Must be at least 18 years of age
- Must be able to regularly lift and carry up to 20 lbs.
- Must be able to occasionally pull at least 40 lbs.
- Must be able to squat, kneel, crawl, crouch, climb, and stoop as part of regular job duties
- Must be able to use hands and fingers to handle or operate objects, tools, or controls
- Required to stand and walk for extended periods
- Must be able to demonstrate proficiency in CPR from floor level
- Vision requirements include close, distance, and peripheral vision
- Must be able to talk and hear
- Complete criminal background Name checked in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
- Drug testing
- Education verification and other credentialing based on position requirements
- Proof of employment history or references (if required)
- Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals
