Job Summary

Coordinate and monitor services and supports for individuals with intellectual and developmental disabilities, serving as the primary individual contributor to address their goals. Supervise Group Home Managers and Direct Care Associates while overseeing person-centered plans, conducting periodic reviews, and initiating adjustments based on feedback. Assist individuals in identifying and communicating requests, review assessments, and ensure quality assurance and state survey improvements. Visit residential homes, vocational centers, and other service locations to assess plan implementation and maintain communication with families and legal guardians. Complete incident reports, employee performance evaluations, and assist in recruiting, hiring, and training staff.

Required Qualifications

  • four-year degree in a human service field
  • two years of experience
  • four-year degree in other fields with four years of experience
  • hold a license in a human service profession
  • Bachelor's degree in Social Work, Social Services, Human Services or Special Education
  • Minimum 2 years experience working with persons with intellectual and/or developmental disabilities
  • 1-year experience if paired with Master's degree
  • Working knowledge of applicable regulations that apply to our service offerings
  • Valid driver's license

  • Ability to lift between 20 and 50 pounds
  • meet the physical requirements of performing CPR
  • Ability to pass a drug screen
  • Ability to pass a background check
  • Name check in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Proof of employment history
  • Proof of references

Desired Qualifications

  • Graduate of a college or university with a Master's degree in a Human Service field
  • Master's degree a plus
  • Supervisory/management experience
  • experience writing person-centered (individualized service) plans
  • Excellent oral and written communication skills

  • Ability to effectively lead and coach others
  • Ability to effectively interview individuals who cannot communicate verbally
