IDD Habilitation Specialist

Job Summary

Conduct Quality Assurance assessments for mealtime, active treatment, and formal programs to ensure effectiveness in adaptive behaviors. Evaluate clients' current levels of adaptive behavior and develop personal outcome programs based on interdisciplinary team priorities. Train home care staff on program implementation procedures and observe their execution to provide feedback and fine-tuning. Write evaluations, training programs, progress notes, and conduct Personal Outcome Interviews to identify outcomes and supports. Review and document formal programs on 30-day cycles to modify as appropriate. Ensure compliance with ICF rules and regulations while mentoring direct support staff.

Required Qualifications

  • High school diploma or G.E.D.
  • Four-year degree in a human service field
  • One year of experience with the population served
  • Regular required to lift 20 lbs
  • Must be able to lift a minimum of 40 lbs
  • Must be able to pull a minimum of 25 lbs
  • Must be able to squat, kneel, crawl, crouch, climb, and stop
  • Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls
  • Must be able to regularly stand and walk
  • Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting
  • Vision requirements include close vision, distance vision, and peripheral vision
  • Must be able to talk and hear
  • Bachelor's Degree in Social Work, Social Services, Special Education, or Psychology or related Human Services degree
  • 1 year of experience working with persons with intellectual and developmental disabilities
  • Valid drivers' license
  • Automotive insurance
  • Ability to read and interpret professional journals, technical procedures, and governmental regulations
  • Ability to write reports and business correspondence
  • Ability to pass background check and drug screen

Desired Qualifications

  • CPR, first aid, NCI and/or role specific certification, a plus
  • Ability to utilize a variety of assessment tools to develop habilitation programs for clients who usually have multiple developmental, physical, or behavior problems
  • Communication skills that foster teamwork
  • Four-year degree in a human service field and one year of experience with the population served preferred
