Assess patient conditions, perform skilled procedures, and implement care plans while collaborating with interdisciplinary teams. Design, direct, and document individualized care plans, including patient teaching and discharge transitions. Prepare patients for treatments and surgeries, report significant changes to physicians, and enforce medication safety protocols. Conduct bedside shift reports with family participation to ensure continuity of care. Maintain a safe work environment by removing soiled equipment and reporting hazards immediately.
