Assess, plan, implement, and evaluate individualized nursing care for a designated group of patients, addressing spiritual, physiological, social, and psychological aspects while collaborating within an interdisciplinary team. Collect and analyze healthcare consumer data including psychological, physiological, sociocultural, spiritual, economic, and lifestyle factors. Set measurable short and long-term goals based on standards of care, modify plans using assessment data and evidence-based practice, and coordinate care, education, and discharge planning. Document all phases of the nursing process in the patient's record and evaluate progress toward identified goals, revising the plan as needed. Requires an RN degree, NE licensure, and BLS/CPR certification upon hire, with ACLS within five months. Located at CHI Health Immanuel in North Omaha.
