Job Summary
Coordinate and manage care for consumers in Integrated Case Management Services through community outreach and home visits to reduce hospitalization risks and promote functional integration. Assist individuals in accessing mental health services, ensuring transportation to appointments, and developing individualized service plans that link them to necessary community resources. Conduct psychiatric emergency interventions and work with multi-disciplinary teams to create discharge plans ensuring continuity of care. Manage consumer wellness, recovery, and dignity while maintaining independence. Requires a Master's or Bachelor's degree in social work or related field with supervisory experience, proficiency in Electronic Health Records, and valid driver's license for essential driving duties. Full-time role at Easterseals New Jersey in Somerset County.
Required Qualifications
- Master's degree in social work, counseling, psychology or related field, from an accredited institution
- Bachelor of Arts in social work, counseling, psychology or related field, from an accredited institution
- One (1) year of relevant supervisory experience
- Three (3) years of post-graduate work experience in mental health
- Skilled in budget management
- Proficient with technology including Electronic Health Records
- Strong verbal and written communication abilities
- Able to manage multiple priorities in a fast‐paced, deadline‐driven environment
- Compassionate, flexible, and client‐focused approach to care
- Valid driver's license
- Ability to drive
