Job Summary
ICM Licensed Medical Social Worker needed by CHNCT to provide social services to members with medical and behavioral health issues, perform assessments, and coordinate care. Requires Master's degree in social work or related field and CT Clinical Social Worker license.
Required Qualifications
- CT Clinical Social Worker’s license must be current and unrestricted
- Must hold a valid and unrestricted state motor vehicle operator's license and have reliable transportation
Desired Qualifications
- Direct experience in handling caseload with chronic medical and behavioral health issues
- Familiar with health care services delivery to economically disadvantaged populations and medically underserved
- Working knowledge of community resources and community service needs of individuals and families in lower socioeconomic levels
- Broad understanding of cultural competency and HIPAA requirements
- Excellent oral and written communication skills
