ICM Case Manager

Job Summary
Provide case management with crisis intervention, discharge planning, and group or individual work for mental health and substance use disorder clients. Initiate referrals to next-level care, coordinate with physical health providers, and advocate for whole-person care while facilitating placements including pre-placement visits and transportation. Complete accurate documentation, meet minimum billable requirements, and provide field-based services as needed. Maintain confidentiality and participate in scheduled program meetings. Shift coverage includes part of the on-call team. This role supports smooth transitions between ACT and ICM levels of care within a client-centered environment focused on improving lives.
Required Qualifications
- Master's degree
- bachelor's degree with four years of experience in social services
- For SUD CM a certification from recognized board
- registration
- negative tuberculosis test (TB)
- valid cardiopulmonary resuscitation (CPR) and first aid certification
- Valid CA driver's license
- current auto insurance per contract requirements
- Cultural competency understanding
Desired Qualifications
- For SUD CM a certification from recognized board is preferred
- Experience providing mental health and/or substance use disorder services preferred
- Bilingual preferred
- Knowledge of Microsoft Office and EHR software preferred
