Job Summary
Assess the health and functional status of members and develop community-based care plans. Participate in the development of an Individual Care Plan and arrange, coordinate, and monitor the provision of appropriate services. Contribute to the determination of a member's need for institutional and community long-term care services. Requires a Bachelor's degree in social work, current LSW licensure, and two years of experience caring for adults 65 and older. Hybrid work environment with a 35-hour standard M-F week.
Required Qualifications
- Bachelor's degree in social work or related field
- Minimum of two years' professional experience in the care of people 65 and older
- Current Social Work license (LSW) or higher
- Ability to obtain information of a personal nature through interviews
- Ability to gain and hold the confidence of clients and their families
- Draw appropriate conclusions and make recommendations
- Knowledge and skill necessary to work successfully within an interdisciplinary team
- Excellent written and verbal skills
- Excellent computer skills
- Proficient with Microsoft Office Suite or related software
Desired Qualifications
- Bachelor's degree in social work or related field highly preferred
- current licensure if appropriate
- Working knowledge of social, emotional, and physical process of aging
