I/DD Care Manager (QP)

Job Summary

Assess members in their communities to build Person-Centered Care Plans and Individual Support Plans that align with their life goals. Facilitate interdisciplinary team meetings to coordinate physical health, behavioral health, and long-term services. Guide individuals through major transitions, such as moving from school to adulthood or returning from care facilities, while educating families on rights and available resources. Serve as the primary navigator for members with Intellectual and Developmental Disabilities, ensuring holistic support across Cabarrus and Union counties. This mobile role requires regular travel to connect directly with clients in their homes and local settings.

Required Qualifications

  • Bachelor's degree in a human services field like psychology, social work, nursing or other relevant human services field
  • minimum of 2 years full-time experience working with individuals with Intellectual and Developmental Disabilities
  • at least 2 years of your work experience was with people with significant Long-Term Services and Supports (LTSS) needs due to their disability in a setting where they receive care in the community
  • Bachelor's degree outside the human services field
  • at least 4 years full-time experience working with individuals with Intellectual and Developmental Disabilities
  • at least 2 years of your work experience was with people with significant Long-Term Services and Supports (LTSS) needs due to their disability in a setting where they receive care in the community
  • Master's degree
  • minimum of 1 year full time experience working with individuals with Intellectual and Developmental Disabilities
  • at least 2 years of your work experience was with people with significant Long-Term Services and Supports (LTSS) needs due to their disability in a setting where they receive care in the community
  • You reside in North Carolina or within 40 miles of the NC border
  • You have the ability to travel regularly as needed to perform job duties
