Job Summary
Manage targeted patient populations by coordinating, facilitating, and monitoring interventions to achieve desired outcomes across inpatient, ambulatory, and post-acute settings. Identify, evaluate, and enroll high-risk patients while performing complete health assessments to develop and implement evidence-based Plans of Care. Collaborate with Primary Care Physicians and interdisciplinary teams to ensure continuity, address care gaps, and facilitate referrals. Perform home visits as necessary to evaluate barriers to self-management goals and coordinate patient education using teach-back methods. Document interventions accurately in medical records and utilize efficiency indicators such as NCQA and HEDIS to improve clinical outcomes and resource utilization.
Required Qualifications
- Bachelor's Degree
- Registered Nurse (RN) with active license in the state of Ohio
- 2-4 years of experience
- 2 years case management
- Must be able to perform any clinical or clerical duty as assigned skillfully
- Must comply with all OSHA and HIPAA regulations
- Must demonstrate strong verbal and communication skills
- Must demonstrate the ability to handle stressful situations appropriately
- Must be able to work flexible hours as needed
- Must understand, adhere to, and implement the Company's policies and procedures
- Must provide excellent customer services skills
- Must consistently display awareness and sensitivity to the needs of internal and/or external clients
- Must take personal responsibility for personal growth including acquiring new skills, knowledge, and information
- Must engage in excellent communication which includes listening attentively and speaking professionally
- Must demonstrate attention to detail and accuracy in work product
- Must set and complete challenging goals
