Hybrid Registered Nurse (RN) Clinical Care Manager - Revere and surrounding area

Community-based registered nurse Care Manager coordinating care for dually-eligible enrollees with complex medical, behavioral, and social needs. The role provides monitoring, follow-up, and clinical care management, integrating health services and community resources to improve health outcomes and quality of life. Responsibilities include engaging with enrollees in homes and community settings, leading the interdisciplinary care team to develop and implement holistic care plans, performing comprehensive assessments (including MDS-HC or successor), guiding care transitions, facilitating access to housing, transportation, and social services, educating members about Medicare/Medicaid benefits, promoting self-management and preventive care, following up after hospitalizations, coordinating with primary care and specialists, advocating for enrollees, and ensuring continuity of care. Requires regular travel; must reside in the assigned Massachusetts territory; flexible hours including evenings/weekends; vaccination requirement; licensure in MA and relevant certifications; eligibility for care management within dual-eligible populations.
