Job Summary
The Clinical Care Manager RN is a community-based role focused on monitoring, follow-up and coordination of care for dually-eligible enrollees with complex medical, behavioral, and social needs in Massachusetts. Responsibilities include in-home and community-based assessments, care plan development, leading an interdisciplinary team, coordinating with healthcare providers and community resources, educating members about benefits (Medicare/Medicaid), promoting self-management, preventing readmissions, ensuring timely transitions of care, and maintaining frequent contact post-hospitalizations. Travel within the territory is required to conduct member, provider, and community visits; the role emphasizes advocacy, alignment with NCQA standards, and collaboration across state agencies and service providers. Licensure in MA as RN/LCSW/LISCW/LMHC is required, with preferred case management certification and a valid driver’s license. Vaccination against influenza is required seasonally for designated positions.
Required Qualifications
- Associates of Science (A.S) degree in nursing required or Master's degree in social work or mental health counseling and independent license required
- Three (3) years of experience as a Registered Nurse/BH Clinician or One (1) year as a Registered Nurse/BH Clinician with two (2) years of experience working with people with complex medical, behavioral and social needs as an LPN, CHW, MA required
- Current unrestricted clinical license in the Commonwealth of Massachusetts as a Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LISCW), or a Licensed Mental Health Counselor (LMHC) required
- Case Management Certification is highly preferred
- Must have valid driver's license, vehicle and verifiable insurance
