Hybrid Registered Nurse (RN) Clinical Care Manager - Brighton and surrounding areas

Community-based Registered Nurse (RN) Clinical Care Manager in Massachusetts coordinates care for dually-eligible enrollees with complex medical, behavioral, and social needs. Leads the interdisciplinary care team to develop and implement holistic care plans, conducts comprehensive assessments (including MDS-HC), coordinates with primary care, specialists, and community resources, and promotes preventive care and self-management. Responsible for ensuring smooth care transitions, reducing hospital readmissions, and assisting enrollees in accessing housing, transportation, food assistance, and other social services. Requires regular travel within Massachusetts; residents must live in the assigned territory and be able to travel for member, provider, and community visits. Must possess a current unrestricted clinical license in MA (RN, LCSW, LISCW, or LMHC), a valid driver's license, and comply with vaccination requirements. Demonstrates strong communication, problem-solving, and collaboration skills to advocate for enrollees and coordinate care across providers and community-based organizations.
