Hybrid Primary Care - Family Nurse Practitioner or Physician Assistant

Provide Transition of Care, Annual Wellness Visits, and same-day primary care visits remotely. Support the clinical team on-site at two Campus Care locations to deliver traditional primary care services, with a medical assistant managing in-person tasks as needed. Perform routine physical examinations, formulate medical and psychosocial treatment plans, and manage referrals in collaboration with physicians. Work full-time weekdays with occasional rotating weekends and no call schedule. This hybrid role expands Hartford HealthCare's virtual care services within its integrated healthcare system.
