Job Summary
Provide high-touch primary care to home-bound and medically complex geriatric patients in Baltimore, Howard County, and Anne Arundel County. Collaborate with the Medical Director and specialists to evaluate, treat, and coordinate care for small panels, while participating in weekly interdisciplinary team meetings. This role includes 20 hours of clinical work and 4 hours of administrative time, with admin duties potentially remote. Night call occurs once per month. The position supports Johns Hopkins Community Physicians' mission to serve Maryland, Virginia, and Washington DC through over 40 locations. Candidates must hold Maryland NP licensure, CPR certification, and a working vehicle.
Required Qualifications
- NP Licensure in the State of Maryland
- CPR Certification
- Must have a working vehicle to travel to patient's homes
- Part time, 20 hours clinical and 4 hours admin
- Night call once per month
Desired Qualifications
- Three to five years NP inpatient or outpatient practice
- General medical experience with geriatric, medically complex and high acuity patients
- 3 years' experience preferred in primary care or acute/urgent care
- Palliative and/or geriatric certification/experience
