Hybrid Interim MDS Coordinator

Assess and determine the health status and level of care for all new admissions, then interview residents and complete documentation based on scheduled assessments. Ensure the accurate and timely completion of all MDS Assessments, including PPS Medicare, quarterly, and annual cycles, while maintaining an accurate schedule with proper reference dates. Coordinate interdisciplinary participation to ensure completeness and thoroughness of documentation as mandated by federal, state, and medical standards. Verify electronic submissions, perform corrections when necessary, and maintain appropriate records through the data entry function. This Full Time Hybrid role requires flexibility to travel between multiple skilled nursing facility locations across Central Illinois as needed.
