Hybrid Float Clinician: Nurse Practitioner (NP), Physicians Assistant (PA), Certified Nurse Midwife (CNM)

Hybrid Float Clinician (Nurse Practitioner, Physician Assistant, or Certified Nurse Midwife) provides direct patient care across PPSNE health centers with telehealth and asynchronous e-visits, performs physical exams and orders/tests (including limited pelvic ultrasound), initiates individualized care plans per affiliate policies, documents in the medical record, participates in patient education and referrals, supports training of Clinic Assistants and staff, and travels to multiple sites as needed. The role emphasizes leadership, collaboration with an interprofessional team, data-informed decision making, adherence to PPSNE policies and standards (HIPAA, CLIA, OSHA), licensure maintenance, and participation in quality improvement and credentialing processes. Essential functions cover patient care, teamwork, patient access, and organizational culture, with flexibility to adapt schedules to patient access needs across sites. Requires active licenses, DEA, BLS, and commitment to PPSNE mission and values. Bilingual English-Spanish preferred.
