Job Summary
Develop and update collaborative practice agreements to improve medication therapy outcomes while providing direct patient care and counseling on dosing, side effects, and interactions. Monitor dispensing accuracy, manage pharmacy technicians, and perform regular audits for controlled substance compliance with DEA and state regulations. Lead multidisciplinary team meetings to discuss complex cases, optimize treatment plans, and participate in quality assurance initiatives. Report adverse reactions, diversion concerns, and operational findings to the Medical Director and Deputy Chief Medical Officer. Ensure all medications are stored and dispensed according to federal standards while implementing facility strategic directives.
Required Qualifications
- Valid and unrestricted license to practice as a pharmacist in the State of Tennessee
- PharmD
- Advanced training (e.g., completion of a pharmacy practice residency or fellowship)
- Board Certification in Ambulatory Care (BCACP) or Psychiatric Pharmacy Practice (BCPP)
- Graduate of an ACPE-accredited pharmacy school
- Previous experience in behavioral health, addiction medicine or ambulatory care setting
