HVC Heart Failure Nurse

RN position within Yale New Haven Health System focusing on heart failure/cardiac care. Provides patient-centered nursing care guided by ANA Standards and the YNHHS practice model, including assessment, diagnosis, planning, implementation, coordination of care, health teaching, and evaluation. Responsibilities emphasize collaboration with patients, families, and healthcare team; adherence to professional standards, ethical practice, cultural congruence, leadership, and ongoing professional development in cardiovascular units (HVC Cardiac Medical Intermediate Care, Cardiac Surgery Intermediate Care, Verdi cardiac units). Requires Connecticut RN licensure, BSN preferred, and current BCLS certification (or obtain within one month of hire).
