HVC Heart Failure Nurse

HVC Heart Failure Nurse (RN) position within Yale New Haven Health System, focused on cardiovascular care across HVC Cardiac Medical Intermediate Care, Cardiac Surgery Intermediate Care/Step Down, and Verdi cardiac units. Provides care according to ANA Scope and Standards of Practice and YNHHS Professional Practice Model. Responsibilities include assessment, diagnosis, outcomes identification, planning, implementation, coordination of care, health teaching, and evaluation; collaboration with patient, family, and health care team; adherence to ethics, culturally congruent practice, and continuing professional development. Requires CT RN licensure; BSN preferred; current BCLS within one month of hire; familiarity with cardiac procedures and telemetry is expected.
