Hub Manager - Care Coordination ( RN ) Per Diem

Coordinate multidisciplinary teams to facilitate safe patient care flow through the continuum by evaluating bed requests, assigning priority beds, and verifying appropriate patient status and level of care. Serve as an expert resource for patient flow, providing ongoing education to staff, physicians, patients, and families while managing direct admits and facility transfers. Liaise across all patient care services to ensure treatment plans are executed, proactively remove barriers to departmental and system-wide flow, and manage hub staffing for assigned and next shifts. Recognize and take immediate action in situations requiring intervention while meeting facility metrics and adhering to Standard Operating Procedures.
