Job Summary
HT Case Manager responsible for direct care to youth 18-24 experiencing homelessness and trafficking. Responsibilities include intake and assessment, development and revision of Individual Service Plans, housing facilitation (including locating housing and assisting with leases), crisis intervention, documentation (HMIS/ETO), case reviews, and group facilitation. Requires coordinating with community resources, providing referrals, budgeting support, and maintaining regular contact with residents. Qualifications include a BA in a related field (preferred), at least 3 years of case management experience with homeless or runaway youth, knowledge of adolescent development and trauma-informed practices, and bilingual English/Spanish preferred. Strong organizational, communication, and collaborative skills, and commitment to trauma-informed, strength-based approaches.
Required Qualifications
- BA degree in Psychology, Sociology, Social Work or related field preferred
- Minimum of 3 years of case management or relevant experience, preferably with homeless or runaway youth
- Knowledge of adolescent development, community resources, and case management skills
- Bilingual English/Spanish preferred
- Strong organizational and communication skills
- Trauma-informed approach
- Ability to conduct home visits and participate in weekly check-ins and case management meetings
