HSE Funded Role - Social Inclusion Liaison Worker

Job Summary
Proactively identify and engage with service users experiencing homelessness, completing comprehensive assessments to prioritize support needs and develop care plans. Coordinate physical and mental health services by acting as a liaison between residents, HSE Primary Care, and specialist providers, while facilitating timely access to appointments. Work collaboratively with Local Authorities and housing providers to support pathways out of homelessness and facilitate successful accommodation integration. Maintain accurate, confidential case records and contribute to service evaluation and quality improvement reports. Deliver services within a trauma-informed, culturally competent framework while participating in supervision and multidisciplinary case discussions.
Required Qualifications
- A recognised third-level qualification (Level 8 or higher) in Social Care, community development or another relevant health or social care discipline.
- A minimum of three years' relevant experience working with vulnerable adults, people experiencing homelessness or individuals with complex health and social care needs.
- Demonstrated experience of coordinating health services to meet the needs of socially excluded groups.
- Knowledge of homelessness, health inequalities, trauma-informed practice and recovery-oriented approaches.
- Excellent communication, interpersonal and relationship-building skills.
- Strong organisational, administrative and report-writing skills.
- Proficiency in Microsoft Office, including Teams, SharePoint and electronic case management systems.
- Ability to work independently while contributing effectively as part of a multidisciplinary team.
- Full, clean Category B driving licence and access to a vehicle for business travel.
