Job Summary
Lead health needs assessments and service development across Galway, Mayo, and Roscommon for individuals experiencing homelessness. Coordinate integrated care responses, manage active caseloads, and build multidisciplinary partnerships with HSE, Local Authorities, and primary care providers. Conduct dynamic risk assessments, advocate for service users, and maintain accurate confidential records in compliance with HSE and GDPR requirements. Prepare governance reports, contribute to quality improvement initiatives, and strengthen pathways between health, housing, and social care services. This integrated care manager role requires a Level 8+ qualification, three years' experience with vulnerable adults, and a Category B driving licence.
Required Qualifications
- A recognised third-level qualification (Level 8 or higher) in Social Care, community development or another relevant health or social care discipline.
- A minimum of three years' relevant experience working with vulnerable adults, people experiencing homelessness or individuals with complex health and social care needs.
- Demonstrated experience of coordinating health services to meet the needs of socially excluded groups.
- Knowledge of homelessness, health inequalities, trauma-informed practice and recovery-oriented approaches.
- Excellent communication, interpersonal and relationship-building skills.
- Strong organisational, administrative and report-writing skills.
- Proficiency in Microsoft Office, including Teams, SharePoint and electronic case management systems.
- Ability to work independently while contributing effectively as part of a multidisciplinary team.
- Full, clean Category B driving licence and access to a vehicle for business travel.
