HRSN Bilingual Support Specialist - Woodburn

Job Summary
Conduct direct outreach, engagement, and tenancy support services for Oregon Health Plan members through the Health-Related Social Needs program. Manage an active caseload by confirming eligibility, calculating assistance amounts, and coordinating rent, utility, and lease-related supports. Communicate with landlords to collect documentation and explain program requirements while entering accurate data into NeedNavigator, Unite Us, and Microsoft Office platforms. Respond to inquiries from members and partners, escalate complex issues, and provide coverage for the Eviction Prevention lobby as needed. Occasional driving in all weather conditions to client sites and partner locations in Marion and Polk counties is required.
Required Qualifications
- High School Diploma or GED
- Two years of case management or social services experience
- Bilingual skill
- Valid driver license
- Insured vehicle available for use on the job
- Satisfactory drive record for the previous three years
- Pass a comprehensive MWVCAA background screening prior to employment
- Pass pre-employment and random drug screenings
- CPR/AED and First Aid Certification within first 30 days of hire
- Knowledge of Housing First principles
- Trauma-informed care
- Person-centered service delivery
- Understanding of social determinants of health and barriers to accessing services
- Familiarity with OHP/Medicaid systems
- HRSN program requirements
- Medicaid Waiver service delivery
- Knowledge of tenant rights
