Job Summary
Support people with moving in and out of accommodation while delivering person-centred support plans to achieve agreed outcomes. Build service users' skills, confidence, and independence through guidance on independent living, budgeting, and maximising income. Assist with benefit claims, rent payments, and ensure compliance with occupancy agreements. Monitor property use, utilities, and address issues promptly, escalating serious concerns as needed. Carry out, record, and review risk assessments and support plans, while participating in 7-day rotas to cover weekends. Maintain accurate records and build effective relationships with partner agencies and external organisations.
Required Qualifications
- Strong knowledge of the welfare benefits system
- Experience in risk assessment and support planning
- Experience working with vulnerable adults/supported living
- Understanding of relevant legislation
- Experience working with Local Authorities and health/social care professionals
- Enhanced DBS required
- 2 Employment References (most recent/current employer & any other previous employer)
- Health and Social Care qualification (desirable)
