Job Summary
Establish effective communication and referral pathways for individuals at risk of homelessness while coordinating treatment for clients with housing needs. Provide wrap-around support in emergency accommodations, prioritizing caseloads based on crisis intervention and severity. Attend multi-disciplinary team meetings to support links with health, social care, and community assets, enhancing partnership working with primary care and voluntary organizations. Work closely with local authorities and Housing Officers to address substance misuse issues and identify clients requiring tenancy sustainment support. Deliver harm minimization training and interventions while assisting users with budgeting, education, and employment engagement. This role requires a full UK driving license and business insurance to work flexibly across sites.
Required Qualifications
- Full UK driving licence

- Use of own car
- Enhanced DBS check
- Right to work in the UK
- An understanding and knowledge of the harmful effects associated with drug and alcohol use in relation to health, social welfare, housing, employment and personal relationships
- Minimum of 12 months experience facilitating successful tenancy sustainment and working to achieve positive community connections
- Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs
- The ability to work with and manage challenging behaviour
