Job Summary

Provide case management services to clients transitioning from emergency shelter or referred from the Coordinated Intake Center, establishing stabilization plans focused on long-term housing stability. Conduct initial and ongoing assessments covering nine areas of concern including health, substance abuse, mental health, employment, legal, benefits, financial literacy, and domestic violence. Deliver regularly scheduled follow-ups to connect clients with community-based services, advocate with landlords to remove housing barriers, and assist with budgeting, income applications, and employment goals. Evaluate client progress against service plan objectives while collaborating with other providers and significant others.

Required Qualifications

  • Bachelor's degree from an accredited college or university in Social Work
  • Three years experience in case management
  • Ability to meet attendance requirements
  • Knowledge of community resources
  • Knowledge of social work concepts and procedures
  • Sensitivity to low-income individuals of diverse backgrounds
  • Ability to work independently and with a team
  • Ability to organize work priorities
  • Ability to work well with the homeless population
  • Ability to handle multiple duties simultaneously
  • Ability to read, write, and communicate the English language
  • Ability to travel to various locations in order to meet with landlords and tour properties

Desired Qualifications

  • Master's degree preferred
  • Background working with the homeless population a plus
  • Bilingual in Spanish preferred
