Job Summary
Assess clients' Health-Related Social Needs (HRSN) including housing stabilization, food security, utilities, transportation, and other social determinants of health for individuals enrolled in NYS 1115 Medicaid Waiver programming. Provide person-centered case management, service navigation, and advocacy to help clients secure and maintain safe, affordable housing while addressing barriers to health and well-being. Ensure all services are trauma-informed, outcomes-driven, and documented for Medicaid reimbursement through Managed Care plans. Work Monday through Friday from 8:30 am to 5:00 pm with occasional night or weekend hours, traveling around the assigned portfolio.
Required Qualifications
- Experience with NYS 1115 Medicaid Waiver programming
- Knowledge of Medicaid Managed Care plans
- Ability to assess Health-Related Social Needs (HRSN)
- Knowledge of housing stabilization
- Knowledge of food security
- Knowledge of utilities
- Knowledge of transportation
- Knowledge of other social determinants of health
- Ability to provide person-centered case management
- Ability to provide service navigation
- Ability to provide advocacy
- Ability to ensure services are trauma-informed
- Ability to ensure services are outcomes-driven
- Ability to document for Medicaid reimbursement
- Ability to help clients secure and maintain safe, affordable housing

- Availability for occasional night/weekend hours
